Provider FAQ

No. Verity HealthNet is not a health plan so we are unable to provide you with information regarding a patients insurance or benefits. For questions regarding patient eligibility and benefit information, you will need to contact your patient's insurance company or health plan administrator directly. That telephone number for benefits, eligibility and pre-certification can usually be found on the back of the patient's ID card.
Because Verity HealthNet does not reprice or process claims in our office, we are unable to provide you with any patient claim information. If a claim issue arises, please contact the customer service phone number listed on the back of the patient's ID card.
You can submit your request on letterhead with the contract holder's authorization via email or by calling 225-819-1133. The request must include the provider’s tax id number and name for this request.

In addition, if your request is for a CPT based fee schedule, we will need an Excel listing of all codes being requested to be sent with the email request.
